I don't have but a few minutes before I leave for work, but just wanted to let you all know our forms are in the mail! Bill's docor's office called yesterday afternoon & I went & picked up his form, then sent both of our's in the mail. They should get them soon. I'm very thankful. We were waiting on this. She just emailed me the "final" draft (minus those 2 forms). Yay! Have a great day!
